How to request verification of enrolment for a Service Canada (CPP) - Declaration of Attendance at School or University.
- Do not mail or drop off your Service Canada form (ISP-1401) to Camosun as the college does not stamp or sign the form. Service Canada will accept a proof of enrolment document (Camosun Letter of Enrolment) to confirm your attendance.
- After your first day of classes, request a Letter of Enrolment through myCamosun. Your Letter of Enrolment will be emailed to you within seven (7) business days of payment.
- See the mailing instructions on the last page of the Service Canada form (ISP-1401).
- Important: Do not complete Section C on the Service Canada form.
- Submit your Letter of Enrolment along with the Service Canada – CPP forms to Service Canada. Camosun does not mail or email letters or forms on the student’s behalf.
For help completing the forms, please contact Service Canada.
